• This form must be completed by Senior Leaders or Directors of the ministry seeking membership.
• Glory City Network membership is also available to itinerate ministers, parachurch ministries and christian organisations and christian based secular organisations.
• All fields should be completed with as much detail as possible.
• By submitting this form you understand and agree with the core beliefs and values of the Glory City Network outlined at https://www.glorycitynetwork.com/statement-of-faith/
• Through the submission of this form you understand, agree and accept your commitment as a network partner to Glory City Church as stated on page 8.
• Glory City Network reserves the right to accept or reject memberships.
